When you click the "Send Request" button, the application will send an email to the email address associated with the User Name. Once you receive the email with a link, simply follow the instructions on that email.

Your "User Name" is UCI# of your child

Your "User Name" is comprised of:
   (1) your Regional Center ID, and;
   (2) your Caseload ID
   Example: 36124A
